Case Management

Case management refers to the coordination of client care and facilitation of needed services on behalf of our participants.

LESHRC case managers provide concrete needs assessment, individual counseling, coordination of health services, facilitation of treatment plans and crisis intervention. Along with these services, our case managers, provide referrals, escorts and  advocacy to enable access to  services such as  legal, medical and dental, drug treatment, shelters, public benefits, among other entitlements.

All services are also provided in Spanish.
